Tattoo Request Form

Please fill out the form to request an appointment for a tattoo. The minimum price for a tattoo is $80. Pricing is determined by size/placement/and length of process. Please be advised that I will not be able to accommodate everyone and will be prioritizing the designs that more suit my capabilities.

I prefer CASH, but can take Venmo, PayPal, Zelle and Credit Cards. You will be asked to send a deposit to secure your appointment once we have discussed the design and set a date. Thank you.
